Acetyl-L-Carnitine

The form of carnitine that crosses into your brain. It fuels your mitochondria, supports acetylcholine production, and pairs with ALA for one of the best-studied anti-ageing stacks.

ALCAR is carnitine with an acetyl group attached — and that small change is everything. Unlike regular L-carnitine, ALCAR crosses the blood-brain barrier, where it fuels your neurons' mitochondria and donates acetyl groups for acetylcholine synthesis. The result: clearer thinking, less brain fog, and better mental energy.

Good for you if: You experience brain fog, want to support mitochondrial health as you age, are building a nootropic stack, or want the classic ALCAR + ALA anti-ageing combination.

What does ALCAR do?

Your brain cells are energy-hungry — they rely almost entirely on mitochondria for fuel. ALCAR helps in two ways: the carnitine portion shuttles fatty acids into your mitochondria so they can be burned for energy, and the acetyl group feeds directly into acetyl-CoA (your cells' primary energy currency) and acetylcholine synthesis.

Regular L-carnitine can't cross the blood-brain barrier, so it only helps muscles and the heart. ALCAR's acetyl group makes it lipophilic enough to get into your brain, which is why it's the form you want for cognitive benefits.

As you age, mitochondrial function declines and carnitine levels drop. This is part of why brain fog, fatigue, and slower thinking are common with age. ALCAR helps counteract this by keeping your neurons' energy factories running efficiently.

What can you expect?

How to take it

Simple protocol

500–1,000 mg in the morning on an empty stomach — food reduces absorption. For the ALCAR + ALA stack, add 300 mg R-ALA at the same time.

For stronger cognitive support, you can go up to 2,000 mg/day split between morning and early afternoon. Avoid evening doses — ALCAR can keep you up.

Start low: Begin at 500 mg/day for the first week to assess tolerance. Some people experience GI discomfort initially that resolves within a few days.

The ALCAR + ALA stack: Take 500–1,000 mg ALCAR + 300 mg R-ALA together on an empty stomach each morning. This combination was shown to restore mitochondrial function in ageing animal models and is one of the most popular anti-ageing supplement pairings.

Which form to buy?

Here's how ALCAR compares to other carnitine forms:

ALCAR L-Carnitine Tartrate
Crosses BBB Yes — gets into the brain No — stays in blood and muscle
Best for Brain energy, cognition, neuroprotection Muscle energy, fat burning, fertility
Acetylcholine support Yes — donates acetyl groups No
Dose 500–2,000 mg/day 1,000–3,000 mg/day
Availability in India Good — multiple supplement brands Very common — widely available

Bottom line: If your goal is brain health, mental clarity, or the ALCAR+ALA stack — you need ALCAR specifically. Plain L-carnitine will not give you cognitive benefits. Look for "Acetyl-L-Carnitine" or "ALCAR" on the label.

Frequently Asked Questions

ALCAR vs L-carnitine — what's the difference?

The key difference is the acetyl group attached to ALCAR. This makes it lipophilic enough to cross the blood-brain barrier — something plain L-carnitine cannot do. Once in your brain, the acetyl group donates to acetyl-CoA (cellular energy) and acetylcholine synthesis (memory neurotransmitter). Use ALCAR for brain and cognitive benefits. Use L-carnitine tartrate for muscle energy, cardiovascular health, and fertility. They are not interchangeable for brain-directed goals.

What does the ALCAR + ALA stack do?

This stack was made famous by Bruce Ames's research at UC Berkeley, where aged rats given ALCAR + R-ALA regained youthful mitochondrial function and cognitive performance. ALCAR provides acetyl groups and carnitine for mitochondrial fuel transport; R-ALA protects mitochondrial components from oxidative damage and improves electron transport efficiency. Together they restore mitochondrial function more effectively than either alone. A typical protocol is 500–1,000 mg ALCAR + 300 mg R-ALA on an empty stomach each morning.

How much ALCAR should I take?

For general brain health and energy: 500–1,000 mg per day. For cognitive support in older adults: 1,000–2,000 mg per day (the dose used in most clinical trials). Take on an empty stomach for best absorption. Start at 500 mg and increase gradually. Take in the morning or early afternoon — ALCAR can cause mild insomnia if taken too late.

How fast does ALCAR work?

Many people notice improved mental clarity and less brain fog within 1–2 weeks. Some feel a subtle energy boost from the first few doses. The full cognitive and neuroprotective benefits build over 4–8 weeks of consistent use. Clinical trials for cognitive improvement typically run 8–12 weeks.

Research & Science

How it works in your body

ALCAR is hydrolysed in the gut to carnitine and an acetyl group. The carnitine component facilitates long-chain fatty acid transport across the inner mitochondrial membrane via the carnitine shuttle (CPT-I and CPT-II). The acetyl group enters the acetyl-CoA pool, feeding directly into the TCA cycle for ATP production.

In neurons, ALCAR's acetyl group also serves as a substrate for choline acetyltransferase (ChAT), supporting acetylcholine synthesis. ALCAR additionally upregulates nerve growth factor (NGF) receptor expression and has been shown to increase mitochondrial membrane potential in aged cells — essentially restoring the "battery charge" of old mitochondria.

What the studies show

Side effects & safety

ALCAR is well-tolerated at standard doses. Watch for these:

Who should be cautious: People on blood thinners (ALCAR may enhance anticoagulant effects), those with seizure disorders (ALCAR may lower seizure threshold at very high doses), and anyone with hypothyroidism (ALCAR may reduce thyroid hormone activity). Check with your doctor if any of these apply.
