Aged Garlic Extract
Aged garlic extract is one of the best-studied natural cardiovascular supplements. It lowers blood pressure, reduces arterial plaque, and doesn't give you garlic breath.
Aged garlic extract (AGE) is garlic that's been aged for 20+ months, converting harsh allicin into stable, odourless compounds like S-allylcysteine (SAC). These compounds lower blood pressure, reduce coronary artery calcium (plaque), and have powerful antioxidant effects.
Good for you if: You have high blood pressure, want to slow arterial plaque progression, or are looking for comprehensive cardiovascular protection.
Dive deeper into the researchCommon side effects
- Mild GI discomfort (less than raw garlic)
- Possible interaction with blood thinners
- Breath/body odour (much less than raw garlic)
What does aged garlic extract do?
Raw garlic's active compound (allicin) is unstable and causes GI irritation and odour. Aging garlic for 20+ months converts allicin into stable, bioavailable organosulfur compounds — especially S-allylcysteine (SAC). These compounds reduce blood pressure, slow coronary artery calcification, improve endothelial function, and act as potent antioxidants.
What can you expect?
- Lower blood pressure — 7–10 mmHg systolic reduction in hypertensive individuals
- Slower plaque progression — reduced coronary artery calcium score progression
- Better cholesterol profile — modest LDL and total cholesterol reduction
- Reduced oxidative stress — powerful free radical scavenging
- Immune support — enhanced NK cell activity, fewer colds
How to take it
600–1,200 mg twice daily with meals. Kyolic brand is the most-studied formulation. For blood pressure, the higher end (2,400 mg/day) shows the best results.
Give it at least 12 weeks for blood pressure effects and 12+ months for meaningful changes in arterial plaque markers.

Is aged garlic extract better than raw garlic?
For cardiovascular benefits, yes. The aging process converts unstable allicin into S-allylcysteine (SAC), which is more bioavailable, better absorbed, and doesn't cause GI irritation or garlic odour. Almost all clinical cardiovascular research uses aged garlic extract, not raw garlic. Raw garlic has antimicrobial benefits that AGE may not fully replicate.
Can aged garlic extract lower blood pressure as much as medication?
Not quite. AGE typically reduces systolic blood pressure by 7–10 mmHg, which is meaningful but less than most antihypertensive drugs (which typically achieve 10–20 mmHg reductions). For mild hypertension (stage 1), AGE may be sufficient. For moderate-to-severe hypertension, it's best used alongside medication, not instead of it.
Does it interact with blood thinners?
AGE has mild antiplatelet activity and can theoretically enhance the effect of anticoagulants like warfarin. However, clinical studies have shown minimal impact on INR at standard doses. Still, if you're on blood thinners, inform your doctor before starting AGE and monitor your INR more frequently.
How long does it take to reduce arterial plaque?
The coronary artery calcium (CAC) studies used AGE for 12 months and showed significantly slower plaque progression vs placebo. Plaque regression (actual reduction) is unlikely with any supplement alone, but slowing progression is clinically meaningful. Think of AGE as a long-term cardiovascular investment.
How aged garlic extract works
S-allylcysteine (SAC) is the primary bioactive in AGE. It inhibits ACE (angiotensin-converting enzyme), improving vasodilation and lowering blood pressure. It activates hydrogen sulfide (H₂S) signalling, which relaxes blood vessel walls. SAC also reduces oxidised LDL, inhibits NF-κB-mediated inflammation, and enhances nitric oxide bioavailability. In arterial plaque, AGE reduces smooth muscle cell proliferation and calcium deposition.
What the studies show
- Blood pressure: Meta-analysis of 12 RCTs — AGE reduced systolic BP by 8.3 mmHg and diastolic by 5.5 mmHg in hypertensive subjects
- Arterial plaque: AGE slowed coronary artery calcium progression by 80% vs placebo over 12 months (UCLA study)
- Cholesterol: AGE reduced total cholesterol by 7% and LDL by 10% in a meta-analysis of controlled trials
- Immune function: AGE increased NK cell activity by 22% and reduced cold/flu severity by 61% in a 90-day RCT
Side effects & safety
- GI discomfort — Much less than raw garlic, but mild nausea or bloating can occur. Taking with food minimises this.
- Garlic odour — Significantly reduced vs raw garlic, but not completely eliminated at high doses.
- Blood thinning — Mild antiplatelet effect. Stop 7–10 days before surgery. Monitor INR if on warfarin.
Which labs to check
- Blood pressure — home monitoring for 4–12 weeks
- Lipid panel — LDL, HDL, triglycerides at baseline and 12 weeks
- hsCRP — inflammation marker that may improve
- Coronary artery calcium score — if available, check annually to track plaque progression
